Student Health Services
Mrs. Rachel Adams, RN, is our school nurse and serves students, faculty and staff by providing care for daily illnesses and injuries, assisting students in the management of chronic health conditions, prepares for health emergencies, oversees the collection, review and storing of student health information, acts as a liaison to community health resources, and promotes overall health and wellness with our students. We are blessed to have her as a resource for our students. Below you will find links to policies as well as downloadable forms for all issues concerning student health.
Brainerd Baptist School Health Forms
Form(s) must be completed at the start of each school year
Over the Counter Medication Permission (will be filled out during registration day)
Request for Medication Administration (complete if medication will be sent to school. This includes EpiPens. Provider signature is required)
Allergy and Anaphylaxis Emergency Care Plan (complete if your child has a severe allergy. Provider signature is required)
Asthma Information & Care Plan (Complete if you child has asthma. Provider signature is required)
Diabetes Emergency Care Plan (Provider specific. Contact school nurse with any questions. Provider Signature is required.)
Cardiac Emergency Care Plan (Complete if your child has a cardiac condition and needs an emergency care plan in place. Provider signature is required)
Seizure Information Form and Action Plan (complete if your child has a history of seizures. Provider Signature is Required.)
Other Medical Alert Form (Complete if your child has a medical condition that the school needs to be aware of that is not listed above)
Student Athlete Permission Form (All students 3rd-5th grade that plan to participate in a sport at BBS. Includes information and forms for both concussions and sudden cardiac arrest. This will be filled out during registration day)
Brainerd Baptist Video Competition
It’s time for the 2nd annual Brainerd Baptist Video contest. That’s right, you can make a video for school!
Last year we focused on the importance of washing our hands. This year we will change things up a bit. Write a song or perform a skit on how we can keep the flu away from BBS. Washing our hands is one component of preventing the flu, but there are several other ways we can keep ourselves healthy this winter. Have a friend or parent video your performance; share your video with me (firstname.lastname@example.org) using google drive or dropbox or another method of your choice.
You may perform alone or grab a group of friends to act with you. Perform with siblings or classmates. Make it a grade event or feel free to mix grades. All people in the video must be current students at Brainerd Baptist, but beyond that the sky's the limit.
A Few Basic Rules:
Video content must be appropriate and “G” rated.
Videos should be no longer than 2 minutes. (Remember, longer is not always better.)
Videos must include at least 2 ways to prevent the spread of germs.
Examples: get your flu shot, wash hands, cover your mouth and nose when you cough, and many more. They are listed on https://www.cdc.gov/flu/protect/habits/index.htm
All participants must be current Brainerd Baptist students.
Deadline for video submission is November 26th. The winning video will be played in chapel and posted on the BBS website. All members of the winning team will receive a special treat from me.
